REGISTERED NURSE (RN) SUPERVISOR
Job Description The Registered Nurse shall supervise the personal care, companion / sitter services, and nursing services offered by the agency. The RN will be the primary supervisor of all medical staff and will review or prepare reports for the client’s physician. The RN shall supervise the provision of personal care tasks in accordance with accepted standards of care. The RN shall be responsible for medically frail clients, which includes elderly, cancer patients, and other ailments that define the client as medically frail.
Responsibilities The registered nurse's responsibilities include, but are not limited to, the following:
Initial provider evaluation.
Periodic evaluation of the client's nursing needs.
Implementation of the Plan of Care.
Assesses physical and mental health needs.
Monitors vital signs as needed.
Observes the functional levels of the client and notes any changes in the physical condition in the client's record / chart.
Distributions of client medication.
Monitors all medications and observes for possible reaction.
Teaches and encourages self-care.
Coordinates care plans with the physician and other service delivery agencies.
Supervises the development and implementation of the care plan.
Preventive and rehabilitative nursing procedures.
Prepares clinical progress notes.
Coordinates with other service providers.
Notifies physician and case manager of changes in the client's condition or additional needs.
Provides health-related education to meet client needs.
Supervises and teaches other personnel.
Coordinates all specialized therapies.
Supervises any services provided for medically frail clients.
In more detail or as an addition to the duties listed below:
Monitoring and supervising the care of those clients whose health status and situation involve complex observations, reviewing referrals from care coordinators, evaluating the client’s needs during initial and subsequent visits, and identifying and assigning the appropriate staff to provide the client care needed.
May assign certain tasks to unlicensed assistive personnel. The registered nurse will utilize the “RN Assignment Decision Tree, generated by the Georgia Board of Nursing, to assist the registered nurse in making appropriate decisions regarding whether to assign a task to an unlicensed person. The RN Assignment Decision Tree assists the registered nurse in evaluating client care tasks on an individual client basis; it guides the nurse in assigning only those tasks that can be safely performed by trained unlicensed assistive personnel.
Developing and revising client care plans as appropriate and reviewing the content of client care plans at least every two months (every 60-62 calendar days).
Supervising Personal Care Assistants and licensed practical nurses.
Completing clinical records, including documenting progress notes and supervisory visit entries, reviewing, signing, and dating all clinical record entries (i.e., service record forms, progress notes) made by the Personal Care Assistants assigned to those clients whose health status and situations involve complex observation.
Review and sign all documentation of the LPN's visits within ten (10) days of each visit and follow up immediately on all concerns raised by the LPN.
Communicating client care needs with other providers, client / client representatives, caregivers, and/or care coordinators.
Making appropriate recommendations to care coordinators regarding client services.
Conducting face‑to‑face visits in the client’s home at least every two months (every 60-62 calendar days) to re‑evaluate the needs of those clients whose health status and situation involve complex observations.
Arranging for orientation and in‑service training of staff, educating the client, client representative, and/or caregiver(s) about the client’s health status, level of functioning, and health‑care needs.
RN – Assignment of Tasks The registered nurse may assign certain tasks to unlicensed assistive personnel. The registered nurse will utilize the “RN Assignment Decision Tree, generated by the Georgia Board of Nursing, to assist the registered nurse in making appropriate decisions regarding whether to assign a task to an unlicensed person. The RN assignment decision tree assists the registered nurse in evaluating client care tasks on an individual client basis; it guides the nurse in assigning only those tasks that can be safely performed by trained unlicensed assistive personnel.
Supervisory functions of the RN include
Conducting an in‑home supervisory visit at least once per year to observe and monitor the in‑home performance of the Personal Care Assistants who provide care to those clients whose health status and situation involve complex observations.
Completing timely, pertinent supervisory notes.
Initial evaluation visit, including development of the written care plan.
Re‑evaluation of the client’s nursing needs.
Initiation of the plan of treatment.
Initiation of appropriate preventive and rehabilitative nursing procedures.
Preparation of clinical and progress notes.
Coordination of services.
Informing the physician, care coordination team, and other personnel involved of changes in the client’s condition and needs.
Client and family teaching.
Supervising and teaching other nursing personnel.
Reviewing and revising the client care plan at least every 62 days and communicating revisions to the appropriate staff.
If nursing services are provided, a copy of the description must be included in the file.
Necessary Knowledge, Skill, and Abilities
At least 4 years of experience working in a supervisory position in charge of medical staff. Good knowledge of the various counseling techniques and intervention strategies effective in rehabilitation. Awareness of the dynamics of group and family interaction; of diagnostic and evaluation procedures useful in the determination of appropriate care modalities; of the physical, social, psychological, and attitudinal.
Working skill in the use of therapeutic individual, group, and family medical techniques; in communication effectively with professional medical, psycho logical, and sociological consultants, facility administrative and supervisory staff, law enforcement, correctional and judicial officials, and community representatives and service providers; in writing evaluative, statistical, and narrative reports.
Working ability to provide leadership and guidance to LPN's and other staff in the care of mental health and physical health. To participate in the evaluation of the quality of services being provided to clients; to develop and implement care plans with appropriate medical, psychological, and/or sociological consultation; to ensure that adequate records are kept secure.
Qualifications
Private home‑care provider registered nurses must have a current license to practice as a registered nurse in the State of Georgia, two years of experience in home health, geriatrics, long‑term care, or a related field. One year of experience in an administrative or supervisory capacity is preferred. Preference should be given to individuals with home‑care experience.
Knowledge of current methods of home management, the provision of personal care, the ability to supervise and coordinate the work of others, to evaluate situations and make decisions, and to communicate and deal effectively with others.
Working knowledge of DCH / HFRD Rules and Regulations for Private Home Care Providers.
