Support Coordinator (Per Diem)

Overview
Department for Persons with Disabilities (DPD) is seeking a dedicated Support Coordinator to work in our Group Homes. This role focuses on support coordination services for adults with intellectual and developmental disabilities.
Shift & Compensation Shift: 7.5 hours per week (Wednesday 8:30 am‑4:30 pm with flexibility for additional days/hours as needed). Hourly Rate: $25.04 per hour.
Responsibilities Manage support coordination services for each participant, including individual discovery, ISP development, plan coordination and plan monitoring. Communicate effectively with supervisors, participants and their families, other service providers and DDD representatives. Produce extensive written documentation and cooperate with agency administration Division/DHS staff during any inspection or investigation.

Requirements Bachelor’s Degree or higher and at least 1 year of full‑time documented experience working with adults (21 or older) with developmental disabilities (paid employment, volunteer experience or family caregiver). Equivalent experience with other populations may qualify if a significant portion involved developmental disabilities. Valid New Jersey driver’s license. Typing speed 40‑60 wpm and strong office/computer skills. Organization skills and ability to operate office equipment. Travel throughout Sussex County and beyond to meet client needs.
