Today, we partner directly with neurology clinics to deliver Chronic Care Management (CCM) as a fully managed service. Using a combination of clinical teams and AI technology, we conduct regular patient phone calls to monitor symptoms, detect early signals of neurodegeneration, and track disease progression over time. This allows neurologists to extend care beyond the clinic visit, improve outcomes for chronic patients, and unlock new, compliant Medicare reimbursement.

Role Description We are hiring a Territory Sales Manager to drive growth by expanding Foresight Health’s partnerships with neurology clinics. This role owns the full sales cycle within an assigned territory, from prospecting and relationship‑building to closing and handoff. You will work directly with neurologists and clinic leadership to help them extend care beyond the visit through a fully managed, AI‑enabled Chronic Care Management program. This is a high‑impact, field‑based role for someone who thrives in fast‑moving healthcare environments and wants to shape the future of chronic neurological care.
Key Responsibilities
Develop and execute a territory sales strategy to meet or exceed revenue targets, identifying priority accounts and mapping out an action plan for outreach.
Proactively build a robust sales pipeline of qualified leads.
Build and nurture strong relationships with key stakeholders at each clinic, including neurologists, practice managers, and C‑suite administrators. Act as a trusted advisor by understanding their challenges in managing chronic neurological patients and aligning our service to their needs.
Manage the end‑to‑end sales process for each deal. This includes scheduling follow‑up meetings and guiding prospects through decision‑making. Provide timely information and respond to stakeholder inquiries throughout the process.
Close deals efficiently and ensure all paperwork (MSA, SOWs, etc.) is completed accurately.
Coordinate with the implementation and account management teams to ensure a smooth handoff of new clients after closing.
Qualifications and Experience
3-5+ years of successful healthcare sales experience
Proven track record of exceeding sales targets in a sales role
Exceptional communication and consultative selling skills
Self‑motivated and organized
Proficiency with CRM systems (e.g. Salesforce) and common sales tools
Valid driver’s license across 1 out of 50 states in the United States
Must be comfortable with up to 80% travel
Nice to Haves
Experience in a startup or high‑growth healthcare company
Knowledge of medical terminology and workflow in clinics
