TAG MedStaffing is hiring a Registered Nurse (Care Management & Coordination) in San Francisco, CA, for our client. This hybrid role involves supervising LPNs, managing Medicare‑compliant care coordination workflows, and developing patient‑centered care plans within a neurology‑focused setting. Ideal for RNs with experience in chronic disease or neurology, who thrive in collaborative and innovative healthcare environments.
Responsibilities as the Registered Nurse (Care Management & Coordination) :
Care Team Leadership : Supervise LPNs delivering care coordination, documentation, and patient engagement in alignment with quality care standards.
Care Plan Oversight : Create, review, and approve care plans to ensure accurate, patient‑centered documentation.
Clinical Quality Assurance : Conduct regular audits to ensure compliance with Medicare PCM/CCM guidelines and maintain high standards of care.
Clinical Liaison : Partner with neurologists to facilitate communication, address clinical concerns, and support comprehensive care delivery.
Process Improvement : Collaborate with operations and AI teams to enhance workflows and support scalable, efficient care models.
Qualifications for the Registered Nurse (Care Management & Coordination) :
Licensure : Active, unrestricted RN license in California required (multi‑state licensure a plus).
Experience : 2+ years in chronic disease management, neurology, care coordination, or related specialty care settings required.
Technical Skills : Proficiency with EMR systems, care management platforms, and digital health tools, Knowledge of Medicare PCM/CCM documentation and billing.
Industry Knowledge : Experience supervising clinical staff, with a background in outpatient neurology, telehealth, or hybrid‑care models; familiarity with neurodegenerative conditions preferred.
Skills & Attributes : Detail‑oriented and dependable, with strong communication skills and a passion for improving patient care and outcomes.
