Essential Functions
Delivery of Patient Care
Assumes responsibility for coordination and delivery of patient/caregiver care.
Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
Develops, implements, evaluates, and updates the plan of care to ensure care is uninterrupted and goals are achieved.
Facilitates patient and caregiver independence to the extent possible.
When providing hospice care, appropriately utilizes VIA Health Partners’ patient‑related protocols, standing orders, and guidelines.
Conducts effective patient and caregiver education and encourages questions and consultation.
Provides education to patients and families as needed.
Communicates effectively with IDG members, patients, caregivers, and other health professionals to ensure continuity of care.
Maintains open lines of communication with supervisor; keeps supervisor apprised of any complex patient/caregiver issues.
Applies principles of infection control and follows guidelines for universal precautions.
Interdisciplinary Group
Attends, participates, and may facilitate interdisciplinary group conferences (IDG) and special care conferences.
Establishes and maintains effective working relationships with team members.
Identifies specific patient/caregiver needs and makes referral to appropriate team members.
Documentation
Ensures accurate, complete, succinct, and timely clinical documentation per VIA Health Partners guidelines and licensure requirements.
Professional and Personal Accountability
Demonstrates competency in knowledge and skills.
Maintains VIA Health Partners’ professional reputation.
Assumes responsibility for professional development, staying abreast of current trends, and incorporating new information into practice.
Maintains current professional licensure, CPR certification, and CEU requirements.
Builds productive and professional relations with external agencies and healthcare providers.
Exhibits time‑management skills, planning the workday while remaining able to respond to crises.
May perform other duties as required
(IPU) Cleans patient rooms per procedure, keeps all facility areas clean and stocked, ensures proper chemicals and protective equipment are used.
Qualifications
Diploma or associate degree in nursing (Bachelor’s preferred).
Current, active, unrestricted compact (multistate) RN license.
Current CPR certification; CHPN preferred.
Minimum 1‑year experience in inpatient units (IPU) or 2 years for other nursing positions; experience in direct care of patients with life‑limiting illnesses required.
Experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, or medical‑surgical settings.
Computer proficiency required.
