Perimeter Behavioral of Forrest City has openings for a Case Manager position. We are a residential certified facility licensed and Joint Commission accredited. We offer adolescents residential services and are committed to improving the lives of those impacted by mental illness.

Responsibilities
Provides case management and support services to the Treatment Team to facilitate clinical management of the cases to which assigned.
Participate as an integral member of the treatment team: attend staff meetings, treatment planning meetings, and assigned committees.
Work closely with the assigned therapist and doctor to coordinate documentation, discharge planning, therapeutic passes, etc.
Communicate with referral sources or other mental health professionals on progress in treatment and involve them in planning.
Coordinate with Primary Therapist, communicate with families about therapeutic passes planning and evaluation.
Participate in quality management.
Identify resident needs (social, legal, family, education) and take appropriate action.
Maintain a positive work atmosphere, promote cooperation and teamwork.
Adhere to organization’s policies/procedures and code of ethics.
Maintain a safe work environment by following safety practices.
Follow policies and procedures regarding patient rights, confidentiality, and privacy.
Accurately complete client records in a timely and legible manner.
Provide detailed information about procedures or treatments to family and other caregivers.
Demonstrate understanding and respect for cultural and religious beliefs.
Recognize and report suspected abuse and neglect according to policy, procedure, and law.
Explain procedures and events in language understood by people served.
Respond to people served based on developmental age.
Adhere to infection control policies and procedures.
Perform other duties assigned as related to the position.
Qualifications
Considerable knowledge of principles of clinical treatment and social casework of children, adolescents, and families in a mental health and substance abuse setting.
Knowledge of child/adolescent growth and development and an understanding of the range of treatment provided by the center.
Bachelor’s Degree preferred in Human or Social Services Field or an Associate Degree with a minimum of two years of experience working with children and adolescents with emotional and behavioral issues. Additional work experience may substitute for a degree.
Schedule 8-hour shift, Monday through Friday (8:00 a.m. to 5:00 p.m.)
Location Forrest City, Arkansas (in person). Reliable commute or willingness to relocate preferred.
