Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekend shifts). We are seeking a full‑time (40 hours per week) provider for a split position between Majestic Care of Hopemont Center in Terra Alta, WV, and Cortland Acres in Thomas, WV. This position can be filled by a nurse practitioner or physician assistant.
The salary range for this position is
$109,000 – $124,000

Responsibilities The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager and collaborates on‑site with the Center Leadership Team to advance center‑specific goals and priorities. The provider works collaboratively with the Medical Director and/or Attending Physician for patient‑specific medical care.
The Advanced Practice Provider offers primary and urgent care services to nursing center residents in partnership with other medical providers. He or she works closely with Center Leadership to achieve AlignMed Partners’ mission of improving lives through high‑quality health care and everyday compassion.
Qualifications • Graduate of an accredited School of Nursing program, licensed as a registered nurse (RN). If a Nurse Practitioner, must hold a Master’s Degree in Nursing with a concentration in Older Adult and/or Family Nurse Practitioner (FNP); other concentrations may be considered based on experience and business need. Physician Assistants must graduate from an ARC‑PA accredited program, pass the PA National Certifying Exam (PANCE), hold state licensure and NCCPA certification.
• Minimum of two years experience working with adult or geriatric populations is preferred.
• In states where practice is permitted, an APRN with an unrestricted professional license and DEA, and a PA with an unrestricted professional license and DEA for each state in which they practice.
