What You’ll Do
Provide Direct Patient Care:
Administer treatments and medications as prescribed, develop individualized care plans, and offer compassionate guidance to patients and families.
Make a Real Difference:
Complete comprehensive assessments, evaluate patient conditions, and ensure care plans are aligned with evolving needs to maintain comfort and dignity.
Collaborate with an Expert Team:
Work closely with physicians, social workers, and other healthcare professionals to optimize patient outcomes and deliver holistic, patient-centered care.
Coordinate Care:
Lead efforts as a case manager for your assigned patients, ensuring seamless, high-quality care across all aspects of the journey.
Teach & Empower Families:
Provide education on self-care techniques, symptom management, and ensure patients remain comfortable and as independent as possible.

What We’re Looking For
Registered Nurse (RN) with a valid license in your state.
Experience in Hospice or Home Health Care:
Minimum of 1 year professional nursing experience; hospice experience required.
Compassionate & Empathetic:
A heart for helping others, especially those facing end-of-life challenges.
Strong Communication Skills:
Coordinate care with physicians, patients, and families, and document clinical notes clearly.
BSN Preferred:
Experience or education in aged, chronically ill, restorative, or psychiatric nursing preferred.
Qualifications
Graduate from an accredited nursing school (BSN preferred).
Valid RN license in the practice state.
Active BLS certification.
A licensed driver with an insured automobile.
