Job Description

Outpatient clinic hours: Monday–Thursday 7 am–7 pm, Friday 7 am–5 pm.

Schedule: 40 hours/week, primarily daytime hours, two evening shifts per week.

The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory and accreditation standards.

Licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by the State of Illinois.

The position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement.

Serves as a role model for all therapists and clinical interns in professional, clinical, communication, education, and leadership activities.

Responsibilities

Demonstrate the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions.

Present a friendly, approachable, professional demeanor and appearance.

Provide accurate information and timely updates to patients and customers; address questions and concerns promptly or identify appropriate resources.

Use effective service recovery skills to solve problems or service breakdowns when they occur.

Maintain primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTAs.

Demonstrate teamwork by helping co‑workers within and across departments, communicating effectively and respecting diverse opinions.

Use organizational and unit/department resources efficiently.

Manage work schedule efficiently, completing tasks and assignments on time.

Contribute to opportunities and processes for continuous improvement.

Participate in efforts to reduce costs, streamline work processes, improve and grow services we provide.

Demonstrate creative problem‑solving and diversity when adapting treatment to special circumstances, evidenced by team feedback and documentation.

Demonstrate essential skill competencies in selecting appropriate evaluation, use of treatment modalities, equipment and protocols; perform re‑evaluations and adjust the treatment plan as required.

Assume complete responsibility for the course of patient management from initial contact to discharge for functional progress, safety and treatment outcome, evidenced by documentation.

Adhere to department policy regarding evaluations, progress notes, and discharge summaries; document all notes in the EMR.

Complete daily notes that reflect patient’s response to treatment and assessment of status, meeting all documentation requirements.

Provide educational materials and methods appropriate to the patient’s level of understanding and motivation.

Participate in committees, department processes, and special projects as per clinical expertise and performance plan.

Work with healthcare teams (physician, nurse, case manager) and participate in internal and external marketing per department expectations.

Create a culture of respectful communication, listening, and timely acknowledgement of contributions.

Accept constructive feedback and make timely changes as evidenced by coaching forms and team feedback.

Proactively complete non‑patient‑care responsibilities within established timelines (coverage notes, PTO/schedule change requests, bonus pay forms).

Contribute to educational efforts of department/clinic (in‑services, journal clubs, new‑hire orientation).

Actively participate in student clinical or observation programs as a clinical instructor, mentor, or observation host.

Maintain productivity standards per department expectations.

Demonstrate willingness to flex schedule or assignment to meet patient care needs and staff meetings outside normal work hours.

Qualifications Required

Bachelor’s Degree in Physical Therapy.

Licensed Physical Therapist in the State of Illinois.

BLS certification from the American Heart Association upon hire.

Preferred

Master’s or Doctoral Degree in Physical Therapy.
