Salary range: $52.00 - $58.00 hourly. Placement within the range is based on experience and internal equity. Union positions follow collective bargaining agreement.

Job Location Lake Forest Hospital (in‑house registry, IHR position).

Description

Minimum shift requirements: 2 weekend shifts and 4 weekday hours per month. Holiday rotation required.

Key Responsibilities

Demonstrate the four NM values of teamwork, patients first, integrity, and excellence.

Present a friendly, approachable, professional demeanor.

Provide accurate information and timely updates to patients and customers. Address questions and concerns promptly.

Use effective service recovery skills to solve problems.

Maintain primary responsibility over patient plan of care through collaboration with treating PT/PTAs.

Demonstrate teamwork by helping co‑workers within and across departments.

Use organizational and unit/department resources efficiently.

Manage work schedule efficiently, completing tasks on time.

Contribute to continuous improvement initiatives.

Participate in efforts to reduce costs, streamline processes, and improve services.

Demonstrate creative problem‑solving and diversity in treatment adaptation.

Select appropriate evaluation and treatment modalities, perform re‑evaluations, and adjust plans.

Assume responsibility from initial contact to discharge for safety and outcomes.

Adhere to department policy on evaluations, progress notes, and discharge summaries. Document in EMR.

Complete daily notes reflecting patient response, performance, and assessment.

Provide educational materials tailored to patient understanding and motivation.

Participate in committees, departmental processes, and special projects.

Develop and maintain professional relationships with healthcare teams (physician, nurse, case manager).

Engage in marketing per department expectations.

Create a culture of respectful communication.

Accept constructive feedback and make timely changes.

Complete non‑patient responsibilities by established timelines.

Contribute to educational efforts: in‑services, journal clubs, orientation.

Actively participate in student clinical/observation programs as instructor or mentor.

Maintain productivity standards.

Show willingness to flex schedule or assignment to meet needs and meetings.

Qualifications

Required

Bachelor’s Degree in Physical Therapy.

Licensed Physical Therapist in State of Illinois.

Basic Life Support (BLS) from American Heart Association upon hire.

Preferred

Master’s or Doctoral Degree in Physical Therapy.
