Work schedule: Monday - Friday with flexible schedules.
Overview As a Registered Dietitian at HCSG, you will manage clinical nutritional services at a long‑term care facility, positively impacting residents and the community.
Responsibilities
Assess residents’ clinical nutrition needs and develop and document individualized plans in the electronic medical record.
Implement and evaluate care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
Provide nutrition education and counseling to residents, families, and staff.
Collaborate with interdisciplinary teams to improve health and well‑being.
Ensure compliance with CMS, state, and federal regulations and company standards.
Provide coverage in multiple buildings as needed.
Perform all other duties as assigned.
Qualifications
Registered Dietitian by the Commission on Dietetic Registration.
Hold a valid certification/license in good standing within the state of practice.
Knowledge of state survey processes and nutrition care regulations (preferred).
Strong organizational and communication skills.
Positive attitude and effective communication with residents, staff, administration, and leadership.
Compliance with COVID‑19 vaccination policies as required.
Valid driver’s license if a multi‑site dietitian.
RD‑Eligible and provisional state licensure accepted in certain locations.
