Position Summary The primary function of your role is to return patients to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a skilled nursing facility (SNF) setting. You will instruct patients in maintaining their achieved level of function after discharge through education, independent exercise programs, and appropriate activity modification, under the supervision of the OT and DOR.
Responsibilities
Assist patients in achieving functional goals as determined by the occupational therapist.
Educate patients on maintaining levels of function post‑discharge.
Implement independent exercise programs and activity modifications.
Maintain accurate documentation and support OT evaluations.
Qualifications
Associate’s Degree from an accredited college.
Current and valid OTA license granted by the state of employment.
Basic computer skills; ability to navigate service delivery software and electronic health records.
Shift & Wage
Full‑time opportunities available.
Shift needed: Varies.
Sign‑on bonus: $5,000.
