At Nexus Family Healing in Manteno, IL, youth in residential care attend Safe Harbor School, an accredited special education program for grades 6‑12, offering individualized curricula with transferable credits upon discharge.

Position Summary:
The LPN provides medical services, procedures and protocols that promote resident health and well‑being.
Primary responsibilities:
Client Centered Medical Care
Monitor acute and chronic medical conditions of clients; provides treatment or consultation as needed
Gather relevant information and assess client status in the event of an emergency situation
Provide appropriate care and/or treatment and direct others as necessary while collaborating to provide and facilitate a referral to higher level of care when appropriate
As needed additional client responsibilities:
Assist with all aspects of the client admissions, and nursing assessments with exception to medical exam
Assess pertinent client data and determine which areas need to be communicated with the physician
Collect and maintain clients' medical data and ensure that medical records are current and accurate including client height and weight
Collaborate with appropriate team members regarding medical aspects of client admissions
Coordinate necessary follow‑up appointments and/or referrals with interdisciplinary teams and communicate medical concerns
Treatment Team Collaboration
Serve as a resource to staff and other interdisciplinary teams regarding client medical concerns
Collaborate with the treatment staff to develop individual client treatment plans
Promotes client health & well‑being
Perform centered health care services, in accordance with the Nexus policies, procedures and protocols
Complete all required training that are department role specific and facility‑wide requirements
Maintain and stay apprised with training and certifications (online training, internal and external, in‑person training, CPR certifications, nursing license, first aid and safety, medication administration, and infection control etc.)
Participate in internal training opportunities with community partners
Client Confidentiality
Ensure client information remains confidential and compliant with Nexus policies involving client privacy, informed consent and confidentiality standards for clients and their families
Implement, monitor and coordinate the administration of client medications
Verify, count, and administer medication
Educate clients on medication side effects, adverse effects, and drug interactions
Monitor clients daily for side effects, adverse effects and or drug interactions
Ensure client prescriptions are filled and the appropriate amount is maintained through each client's stay within the resident milieu
Documentation of medication administration in electronic health record
Monitor medication logbooks for completion and accuracy as needed
Requirements:
Required Education and Licensure:
Graduate of an accredited school of Practical Nursing
2+ years' experience in a clinic and/or hospital environment
Valid driver's license required. Must meet state regulating agency and Nexus Home Office driving requirements
Preferred Education and Experience:
4+ years' experience in a clinic and/or hospital environment
