Base pay range $44,000.00/yr - $51,000.00/yr

Position Summary Coordinate eligibility and intake process for persons served. Facilitate the development of the Comprehensive Consumer Service Plan (CCSP). Assist persons served in obtaining appropriate services and crisis intervention. Mediate conflict related to the services for persons served; maintain master records, correspondence, and all other required data and advocate for persons served.

Essential Job Functions

Process requests for services by obtaining necessary assessments, conducting intakes, completing social histories, and determining service eligibility for persons served.

Prepare for and facilitate the Interdisciplinary Team in developing the Comprehensive Consumer Service Plan utilizing positive verbal communication, human relations, and teamwork skills.

Advocate and/or assist persons served in receiving services identified in the planning process.

Ensure regular face-to-face and collateral contact with persons served, providers, family/guardians, and other team members.

Assist persons served/families/guardians to maximize their abilities for self-direction, by enabling and empowering them to the greatest extent possible.

Qualifications

Bachelor’s degree from an accredited college or university with a major or at least thirty (30) semester hours or its equivalent in the behavioral sciences, education, health care, human service administration, or the social sciences required. A minimum of one (1) year related successful work experience in the delivery of human services, specific to the needs and abilities of the populations being served; or have an Iowa license to practice as a registered nurse with three years of experience in the delivery of nursing or human services to the population group is required.

Possess the ability to consistently exercise discretion and independent judgment.

Must meet agency policy on transportation.

Possess the ability to work on a daily basis utilizing standard office equipment.

Proven proficient ability to implement agency rules and procedures using verbal, presentation, and written communication skills, human relation skills, team working skills, and organizational skills.

The position will contain Medium physical work consisting of occasional 2-hand lifting from floor to waist of up to fifty (50) pounds and occasional 2-hand push/pull of up to sixty (60) pounds of force. This position consists of frequent standing, walking, and reaching.

The position requires frequent forward bending crouching or stooping.

The employee will need to occasionally climb stairs without the use of railings and occasionally sit.
