Position Overview Join us as a Licensed Practical Nurse (LPN) and be at the heart of compassionate hospice care. In this field-based role, you’ll support patients in their homes and communities, helping bring comfort and connection when it matters most. As patient volumes grow, this position offers the opportunity to expand into full-time work—perfect for a dedicated LPN ready to grow with our team.

Key Responsibilities

Provide direct clinical care including symptom and pain management, vital signs, medication monitoring, and wound care

Collaborate with the interdisciplinary care team—physicians, social workers, therapists—to develop and implement personalized care plans

Offer emotional support and education to patients and families navigating end-of-life care

Maintain accurate and timely documentation in compliance with hospice standards

Be available 24/7 for on-call needs, including nights, weekends, and holidays, to ensure seamless patient support

Qualifications

Active

LPN license in Minnesota

Prior experience in

hospice, palliative, nursing home, or home health

settings is required

Strong clinical judgment paired with empathy, patience, and excellent communication skills

Comfortable working autonomously in patients’ homes and coordinating care across disciplines
