Provide expert clinical care to patients with life‑limiting illnesses Act as the point person for patients and families navigating hospice Manage medication and symptom relief, ensuring comfort and dignity Collaborate with social workers, chaplains, aides, and physicians Educate caregivers and guide them through emotional and logistical challenges Maintain up‑to‑date, accurate documentation Qualifications
Active Minnesota RN license 2+ years of hospice, oncology, or palliative care experience Passionate about patient‑centered, compassionate end‑of‑life care Strong interpersonal, critical thinking, and decision‑making abilities Commitment to ethical, regulatory‑compliant practice Veteran Friendly Employer
