Registered Nurse, Home Health

(acquired by Humana in 2021) is seeking a dedicated Registered Nurse to provide skilled nursing visits in patients’ homes under the direction of the treatment plan established with the physician and input from patient/caregiver.
Base pay range $77,200 – $106,200 per year
Responsibilities
Provide initial and ongoing assessment of client needs using OASIS data incorporated into the Comprehensive Assessment.
With input from patient/caregiver and in conjunction with the physician, formulate and implement a plan of care.
Evaluate the effectiveness of the care plan and make necessary adjustments.
Provide emotional and physical comfort and safety of the client, taking into consideration their rights and cultural background.
Receive and transcribe physician orders.
Notify the physician and agency supervisor of unusual reactions and/or changes in the client’s condition.
Document all appropriate observations and treatments in compliance with agency policies and procedures.
Participate in case conferences, team meetings, staff meetings, and performance‑improvement activities as assigned.
Provide supervision for Licensed Practical Nurses and/or Home Health Aides as assigned.
Provide any skilled nursing service for which the nurse is appropriately trained and prescribed under the plan of care.
Provide a monthly summary of skilled services and client outcomes to the physician and agency supervisor in compliance with agency policies and procedures; provide information for 60‑day progress reports.
Adhere to all agency policies and procedures, including the HIPAA privacy rule.
Maintain strict confidentiality of all patient, employee, and agency information.
Perform other duties as assigned by the supervisor for which the nurse is qualified.
Regulatory Requirements
Graduate of an approved school of nursing.
Currently licensed in the State of Texas.
Minimum one year of current experience in the health‑care community; home‑health experience preferred.
Excellent verbal communication skills.
Professional Requirements
Knowledge of Medicare and Medicaid guidelines.
Excellent communication skills and ability to establish good rapport with other staff members.
Working knowledge of home health care and the principles and techniques of professional nursing and required documentation.
Skillful in organization of work and time management; knowledge of management processes.
Ability to contribute to the quality of care through constructive communication with nursing managers and staff.
Schedule 40 hours per week.
Pay Range The compensation range below reflects a good‑faith estimate of starting base pay for full‑time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job‑related skills, knowledge, experience, education, certifications, etc.
