Weekend Admissions Nurse (RN) - Home Health
Stella Maris Inc. provides compassionate, skilled home health care for adults in central Maryland. We offer services that allow patients to manage their medical situations in the comfort of their homes.
Shift: Friday, Saturday, Sunday 8:00 AM – 6:30 PM.

Responsibilities
Main point of contact to answer admissions questions about home health services to prospective patients and families.
Explain insurance benefits and coverage, patient responsibilities, and alternative payment options.
Coordinate the admissions process to the appropriate program and complete paperwork promptly and accurately.
Develop the initial plan of care based on input from patient, family and physician.
Provide information and education to physicians, discharge planners, and community organizations about Stella Maris Home Health services.
Requirements
RN licensure in the State of Maryland.
Prior experience in home health required.
Valid driver’s license and reliable transportation.
OASIS experience required.
This position requires daily travel in the assigned region (Baltimore County & Baltimore City).
