Wellness Home Services Licensed Practical Nurse (LPN)
Join our team at Acts Retirement-Life Communities as a Licensed Practical Nurse (LPN) and help deliver individualized, one‑on‑one care to our residents. Working under the direction of a Registered Nurse (RN) and in compliance with state guidelines, you will manage chronic conditions, support post‑operative recovery, and address complex medical needs while ensuring safety, dignity, and comfort.
Responsibilities
Provide medication management and vital sign monitoring.
Collaborate with multidisciplinary care teams to develop and implement care plans.
Offer education and guidance to residents and families regarding health conditions and care procedures.
Support residents in maintaining independence and improving quality of life.
Qualifications
Active LPN license in good standing with the applicable state Board of Nursing.
Minimum 1–2 years of clinical nursing experience, preferably in acute care or home health.
Current CPR certification (or obtaining within three months of employment).
Valid state driver’s license and proof of current automobile liability insurance if transportation of residents is required.
Pay Range $24.38 – $35.79 per hour. Starting rate will vary based on skills and experience.
