RN/LMSW Case Manager, Inpatient Rehabilitation
Position at Lifepoint Health® in partnership with Peak Rehabilitation Hospital, a joint‑venture between Duke Health, WakeMed, and Lifepoint Rehabilitation. This full‑time role focuses on delivering high‑quality inpatient rehabilitation services.
Responsibilities
Completes departmental orientation, initial and annual competencies.
Assists with departmental performance‑improvement initiatives, collecting and reporting data as requested by the supervisor.
Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care or discharge, and develop solutions.
Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
Schedules family conferences and/or communicates with caregivers following each team conference, and more often as needed to keep patient and caregiver informed of progress and discharge plans.

Qualifications and Requirements
Current Registered Nurse or Social Work licensure or licensure as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, or RN‑BC).
Minimum of 2 years social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
Excellent organizational, time‑management and analytical skills.
Strong teamwork orientation and cooperative approach.
