Case Manager – Rehabilitation
Dignity Health East Valley Rehabilitation Hospital – Gilbert, operated jointly with Lifepoint Health and Dignity Health, is committed to providing superior patient care. As a Case Manager, you will provide care coordination, conduct needs assessments, identify care options, and communicate with patients and families within an interdisciplinary environment.

Qualifications
Current Registered Nurse or Social Work licensure or licensure in Respiratory Therapy, Physical Therapy, Speech‑Language Pathology or Occupational Therapy.
Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, ANCC RN‑BC).
Minimum two years of social work or case management experience in an inpatient setting, with acute/rehabilitation hospital experience preferred.
