The position is focused in a Skilled Nursing Facility (SNF) setting, where you would serve as a clinical leader, working collaboratively with the facility’s care team to oversee both short‑ and long‑term residents. Responsibilities include admissions, discharges, monthly stability checks, and evaluations for changes in condition. You’ll also handle advanced care planning, comprehensive assessments, and prescribing treatments.
