Hours:
Monday - Friday, 8:30 AM - 5:00 PM (flexibility), 1‑2 weekends per month.
Overview The Fertility Care Patient Navigator is a compassionate, service‑minded role that serves as the primary point of contact for patients, guiding them through their fertility journey and working closely with a multidisciplinary team to ensure exceptional care and support.
Key Responsibilities
Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow‑up care.
Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
Oversee treatment coordination across departments for individual patient care plans.
Maintain accurate, up‑to‑date patient records in the electronic medical record (EMR) system.
Provide emotional support and resources to patients and families, addressing concerns or barriers to care.
Educate patients on treatment protocols, self‑care strategies, and medication usage.
Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
Advocate for patients’ needs and help resolve any issues or barriers to care.
Other duties and projects as assigned.
Position Requirements
Education:
High school diploma required. Bachelor’s degree in healthcare, social work, or related field preferred. Certified Medical Assistant Certification (CMA) preferred.
Experience:
Minimum 1‑2 years in customer service, patient navigation, case management, or related healthcare role. IVF/fertility experience and background in women’s health a plus.
Skills:
Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
Excellent communication, organizational, and problem‑solving skills.
Excellent time and people management skills with the ability to work independently, multitask, and prioritize work.
Proficiency in using electronic medical records (EMR) and healthcare software.
Empathy and a patient‑focused approach, ability to work collaboratively in a multidisciplinary team.
Bilingual skills are a plus.
Hourly Rate:
$24.00 - $26.00 per hour (final offers based on experience, skills and qualifications).
