LIFE COORDINATED is an integrated care company and PACE Program (Program of All Inclusive Care for the Elderly) and this is the way we operate. A Dietary Member Support Service is integral to delivering comprehensive and integrated care. We are seeking a Dietary Member Support Service to join our team that wants to be an integral part of a team that is changing lives every day. We are passionate and purpose driven to be best-in-class.
Our goal is to help our members live their best life based upon their personal goals.
PACE allows us to invest in the right things to address root cause and environmental factors, not just symptoms and resulting medical care.
Licensure / Certification Requirements
Current KY Food Handlers Certificate
Current, valid driver’s license
Experience Desired or Preferred Knowledge, Skills and/or Abilities:
a. Knowledge of food preparation and safety
b. At least one year professional experience working with an elderly or frail population.
c. Working knowledge of the administrative organization of community facilities.
d. Skill in the application of current procedures and techniques of patient care.
e. Ability to plan and coordinate meals for individuals, families, and groups under the guidance of supervisor.
f. Ability to communicate effectively.
g. Ability to establish and maintain cooperative working relationships.
h. Ability to accept and utilize guidance.
i. Ability to perform duties in accordance with established policies and procedures
j. Acceptable physical and mental health to carry out the responsibilities of the position
General Responsibilities
a. Knows and adheres to the philosophy and goals of (PACE Program).
b. Assist with preparing meals according to menus and dietary restrictions.
c. Wash, peel, chop and portion feed items
d. Prepare beverage snacks or supplements as required
e. Serve
f. Clean kitchen dining areas utensils, dishes, and equipment
g. Dispose of garbage properly and maintain sanitary wok conditions.
h. Monitor inventory levels and notify supervisor when supplies run low
i. Knowledge of food safety
j. Maintains a quiet and safe environment for clients, visitors, and staff.
k. Keeps confidentiality of client records, reports, and discussions.
l. Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
m. Utilizes supplies and equipment economically.
n. Informs the Clinical Services Director of “unusual incidents.”
o. Maintains timely and quality documentation of services provided
p. Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Clinical Services Director as appropriate; responds in a positive manner to constructive criticism; serves as a role model for students and staff members
