Base Pay Range $50.00 per day – $125.00 per day
Pay Details $100.00 per Oasis visit with an additional $25 bonus if notes are completed within 24 hours; $50.00 per follow‑up/revisit with an additional $25 bonus if notes are completed within 24 hours.
Job Summary As a Registered Nurse, you will play a vital role in planning, organizing, and delivering home care services. You will be responsible for assessing patients, developing care plans, coordinating with other healthcare professionals, and providing education and support.
Responsibilities
Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status; develop and implement individualized care plans.
Medication Administration: Administer medications and treatments as prescribed; monitor for adverse reactions.
Wound Care: Provide wound care management, including dressing changes and infection control.
Patient Education: Educate patients and families about medical conditions, medications, and self‑care techniques.
Monitoring and Evaluation: Monitor patient progress; evaluate effectiveness of care plans and adjust as needed.
Collaboration: Coordinate care with physicians, therapists, social workers, and home health aides.
Documentation: Maintain accurate, up‑to‑date medical records of assessments and interventions.
Support and Advocacy: Provide emotional support and advocate for patient rights and preferences.
Qualifications
Graduate of an approved school of nursing or Bachelor's degree in Nursing from an accredited college/university.
Currently licensed, in good standing, to practice in New York as a Registered Nurse.
Minimum 1 year community health nursing experience or equivalent professional experience.
Knowledge of nursing and medical practices, procedures, regulations, and guidelines pertaining to home care, including OASIS.
Compliance with accepted professional standards and practice.
