Company Overview Caregiver is a leading Texas-based provider of services to adults with intellectual and developmental disabilities (IDD) that supports clients with the design, delivery, and management of personalized treatment plans to help them lead their best lives. We are looking for a Home Coordinator to join our team!

Job Overview The Home Coordinator is responsible for providing direction and guidance to direct care staff caring for clients with intellectual and/or developmental disabilities in a supported living or community-based environment. This includes responsibility for the activities of staff and the whereabouts of all clients under their supervision at all times, as delineated in the ISP and ensuring that required documentation is completed accurately, thoroughly and timely. Incumbents are on call twenty-four hours a day and must respond in a timely manner to unexpected events and/or emergencies with clients under their care. The Home Coordinator is also expected to provide direct client care.

Essential Duties And Responsibilities

Plans, organizes, and directs the care of clients.

Supports related to outcomes and the support and supervision of the individual to assure that the overall supports provided are in adherence with the individual’s person-centered plan.

Monitors the client care activities of direct support staff.

Monitors Individual Support Plans and delivery of client care plans as appropriate; takes actions to ensure Standards of Care are met; participates in quality improvement process.

May assist with determining staffing schedules, staffing levels, and staff schedules in coordination with the Program Manager.

Assures that staffing patterns developed are in adherence with the team recommendation and PAWS Allocation.

Responsible for preventing client abuse by adhering to all policies and procedures, attending periodic training, reporting suspicious and inappropriate behavior, and following mandatory abuse reporting requirements.

Completes administrative and clerical documentation including but not limited to reviewing daily logs and notes detailing client activities supporting the approved Individual Support Plan (ISP) that support services billed through Medicaid HCB Waiver Program.

Assures safe operation of all equipment in the house by completing routine site monitoring reports.

Plans and organizes daily activities in accordance with Individual Support Plan.

Provides direct client care, including transportation, maintenance of household inventory, menu planning and preparation, etc.

Provides weekly oversight of individuals’ finances by providing direct assistance with managing personal funds (personal ledger, petty cash in home and receipts). Reports any signs of misappropriation, exploitation, or financial impropriety regarding individual funds to the County Board through Unusual Incident (UI) reports and to the direct supervisor (PM).

Maintains and schedules medical appointments. Completes all follow-ups including maintaining the appointments in Advisor and attaching related documents.

Other duties as assigned.

Qualifications

One-year supervisory experience working with developmentally, intellectually and/or physically disabled individuals preferred.

Licenses, Certifications, and Registrations

Valid Indiana Driver’s License Required

Valid Automobile Insurance Required

CPR and First Aid. (Provided if not already acquired)

Knowledge And Skills

Requires extensive mental activities including the ability to use educated and intuitive judgment, advise, counsel, influence, debate, negotiate, organize, plan, and synthesize concepts.

Ability to work in a demanding environment.

Must be able to make wise decisions under stressful circumstances.

Requires exceptional coaching and mentoring skills to improve employee development.

Must be proficient in Microsoft Office including Word and Excel. Equipment skills required: PC, calculator, copier, fax machine, scanner.

Duties of this job are performing the following

Sitting is 50% of work time.

Standing, and walking is 30% of work time.

Bending, pushing, pulling, stooping, reaching, and kneeling is 30% of work time.

Grasping and finger/hand manipulation is 50% of work time.

Ability to lift up to 50 pounds.

Ability to operate computer, facsimile, copier, shredder, and calculator.

An office setting, varying degrees of background noise, standard office lighting and ventilation, occasional temperature fluctuations are found in the work environment.

Normal business hours with flexibility to work overtime or non-standard hours as required to maintain staffing in the assigned home for continuity of care.
