Job Overview The Home Coordinator is responsible for providing direction and guidance to direct care staff caring for clients with IDD in supported living or community‑based environments. The role includes supervising staff activities, ensuring documentation, being on call 24/7, responding to emergencies, and providing direct client care.

Essential Duties And Responsibilities

Plans, organizes, and directs the care of clients.

Monitors the client care activities of direct support staff and ensures adherence to individual support plans and Standards of Care.

Assists in determining staffing schedules and levels in coordination with the Program Manager.

Prevents client abuse by following all policies, attending training, reporting incidents, and meeting mandatory reporting requirements.

Completes administrative and clerical documentation, including daily logs and notes supporting the Individual Support Plan and Medicaid HCB Waiver Program billing.

Ensures safe operation of equipment in the home by completing routine site monitoring reports.

Plans and organizes daily activities in accordance with the Individual Support Plan.

Provides direct client care, including transportation, housekeeping, menu planning, and preparation.

Provides weekly oversight of individuals’ finances and reports any signs of financial impropriety.

Maintains and schedules medical appointments and follows up with relevant documentation.

Performs other duties as assigned.

Qualifications

High School Diploma or GED required.

Preferred: one year of supervisory experience working with developmentally, intellectually, or physically disabled individuals.

Required to complete 60 course hours of DODD training for competency.

Licenses, Certifications, and Registrations

Completion of an accepted Ohio medication certification and Ohio DODD eight‑hour training course.

Valid Ohio Driver’s License.

Valid automobile insurance.

Proof of completed High School or GED (Diploma or Official Transcript).

CPR and First Aid certification.

Knowledge And Skills

Strong judgment, counseling, influencing, debating, negotiating, and organizing abilities.

Ability to work in a demanding environment and make wise decisions under stressful circumstances.

Exceptional coaching and mentoring skills to improve employee development.

Proficiency in Microsoft Office (Word, Excel), PC, calculator, copier, fax machine, scanner.

Understanding of physical demands and work environment; reasonable accommodations may be made.

Physical Demands and Work Environment

Sit 50% of time, stand/walk 30%, bend/push/pull/stoop/reach/kneel 30%.

Grasping/finger manipulation 50% of time.

Can lift up to 50 pounds.

Operate computer, facsimile, copier, shredder, calculator.

Office setting with normal business hours; flexibility for overtime or non‑standard hours as required.
