Position Summary This position is responsible for assessing, documenting, and monitoring the overall functional, physical and behavioral health status of members assigned to them. Based on the assessments, the case manager collaborates with the member and support system to develop a service plan that meets member needs in the most cost‑effective and integrated setting.
Core Functions
Primary contact for the ALTCS member, explaining the program, rights, responsibilities, grievance and appeal system, and other information per regulations.
Comprehensively assess and document the member's biopsychosocial functioning within AHCCCS time frames, identifying strengths and needs.
Develop and implement a service plan based on the member’s strengths, needs, and placement preferences, authorize and coordinate with provider agencies.
Assist the member in defining personal goals, identify barriers, and encourage resolution.
Act as facilitator and/or advocate for the member in dealing with providers, community programs, or other organizations.
Act as gatekeeper to ensure the member receives the appropriate, cost‑effective services in the appropriate setting.
Work within budgetary allowances; collaborate with nursing management, medical staff, interdisciplinary team, physicians, payers, community agencies, provider networks, and regulatory agencies.
Minimum Qualifications Knowledge, skills, and abilities normally obtained through completion of a bachelor’s degree in social work and two years of experience serving elderly persons and/or persons with physical disabilities or those determined to have a Serious Mental Illness (SMI).
Preferred Qualifications Bilingual skills preferred in some assignments.
