Banner Plans & Networks (BPN) integrates Medicare and private health plans to reduce healthcare costs while keeping our members in optimal health. It offers remote and hybrid work settings.

We service the Arizona long term care AHCCCS population, case‑managing beneficiaries to ensure services are identified and authorized according to members' person‑centered assessments.

Position Summary This position is responsible for assessing, documenting and monitoring the overall functional, physical and behavioral health status of members assigned to them. Based on the assessments, the case manager, collaborating with the member and his/her support system, develops a service plan that meets member needs in the most cost‑effective and integrated setting.

Core Functions

Primary contact for the ALTCS member, explaining the program, members' rights and responsibilities, grievance and appeal system, and other information per regulations.

Comprehensively assess and document the member’s bio psychosocial functioning per AHCCCS time frames, identifying strengths and needs.

Develop and implement a service plan based on the member’s strengths, needs, and placement preferences; authorize and coordinate with provider agencies.

Assist the member to define personal goals, identify barriers, and encourage resolution.

Act as a facilitator/advocate for the member with providers, community programs, or other organizations.

Act as a gatekeeper to ensure the member receives appropriate, cost‑effective services in the most suitable setting.

Facility‑based while staying within budgetary allowances; internal customers include nursing management, staff, medical staff, and the interdisciplinary healthcare team; external customers include physicians, payers, community agencies, provider networks, and regulatory agencies.

Minimum Qualifications

Bachelor’s degree in social work.

Two years of experience serving elderly and/or persons with physical disabilities or with a Serious Mental Illness (SMI).

Preferred Qualifications

Bilingual language ability (preferred in some assignments).
