We are seeking a
Licensed Therapist
to join our team at our program in
Sacramento, CA .
As a member of the Intensive Transition Services (ITS) Team, the Licensed Transition Care Coordinator is qualified in the human services field and possesses the skills and experience necessary to provide clinical therapeutic interventions, care coordination and supportive services to program individuals with a mental health diagnosis and Intellectual/Developmental Disability (Dually Diagnosed). Services are provided in a multi-disciplinary team setting and done under the clinical supervision of the Program Director.
Job Description
Provide and collaborate on comprehensive goal plans with individuals and their families.
Coordinate service activities, including treatment plan development and implementation.
Serve as a liaison with community agencies and families to maintain treatment coordination.
Educate and support families, and advocate for individual rights.
Deliver treatment, rehabilitation, and support services within a team, participate in meetings, and perform necessary cross‑training as directed.
Offer individual support therapy and promote social skill development in community settings.
Offer practical assistance, advocacy, individualized support, and supervision to help individuals with daily living necessities.
Earn up to $105,000 annually + mileage reimbursement
