Reasons Eating Disorder Center (Reasons) is seeking professional candidates, preferably with previous experience working with eating disorders, trauma, substance use and LGBTQIA+ communities to provide compassionate care to both adult and adolescent patients in our inpatient eating disorder program at
BHC Alhambra Hospital .
The Case Manager for Reasons is responsible for managing the social work and discharge planning coordination offered to the Reasons patients, which may include clinical screening and assessment of new admissions, input into treatment plans, interfacing with the treatment team / external case managers, authorizations, and utilization review with managed care organizations. This position also provides resources, implements and completes all aspects of the discharge planning process, provides aftercare planning, relapse prevention, and life‑skills therapy groups on schedule. The position will attend treatment teams, audit medical records, and meet with patients and families to coordinate discharge planning and social work needs.
Key Responsibilities Include:
Ensure patient attends the appropriate level of care or program as indicated on the treatment plan, or as needed when there are changes in the patient’s status.
Maintain necessary documentation of all services and the quality of care.
Track and complete all utilization reviews, working with third‑party payors regarding certified days.
Contribute to treatment planning.
Coordinate discharge plans with the patient/family/significant other as identified by the treatment team.
Provide, monitor, and document discharge planning activities including identification of discharge needs, involvement of patient, family, and significant others, and coordination of referrals to other agencies.
Requirements Education:
R.N. or Masters or Doctorate Degree from an accredited college or university in social work, mental health, psychology or a related field required.
Experience:
A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable license. One year of experience working in an eating disorders treatment setting preferred.
Additional Requirements May be required to work overtime and flexible hours. Bilingual Spanish – preferred. Experience in telehealth and electronic medical record documentation.
