Position: Licensed Practical Nurse (LPN) / Registered Nurse (RN) Position Summary

The licensed practical nurse (LPN) or registered nurse (RN), reporting directly to the Director of Nursing, is responsible for delivering, coordinating, and evaluating resident care in alignment with physician orders, individualized service plans, and regulatory standards. This role combines hands‑on clinical responsibilities—medication administration, assessments, documentation, and direct resident support—with leadership functions such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leadership.

Essential Functions

Provide services in accordance with nurse practice act and state/federal nursing center regulations.

Work with the Director of Nursing to provide direction and oversight to the care staff regarding the resident’s needs.

Review and revise assessments and care plans at admission, periodically per regulation, and at changes in resident physical or psychosocial condition.

Review and transcribe physician's orders and provide medical treatments as ordered, assisting physician with resident examination as needed.

Initiate a resident education plan according to individualized needs, as prescribed by physician and/or community policy.

Observe residents and communicate condition changes to physicians, health care proxies, and supervisors, providing timely interventions.

Administer and document medications in accordance with the five rights: right medication, right route, right resident, right time, and right dose.

Supervise and monitor medications for residents who self‑administer, if applicable.

Document all pertinent observations, red flags, incidents, interventions, and clinical procedures throughout the shift.

Assist and monitor residents with meeting nutritional/hydration needs and document appropriately.

Assist residents with activities of daily living such as bathing, laundering, oral care, toileting, dressing, etc.

Assist residents in scheduling medical, dental and laboratory appointments.

Utilize proactive intervention, monitoring, and safety in rooms and rounds.

Answer residents' call lights promptly.

Assist residents with transfers and ambulation as directed, using proper body mechanics.

Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess the condition of resident.

Maintain a restraint‑free environment at all times.

Support fall‑prevention programs for all residents in need.

Respond to life‑saving situations based upon nursing standards, policies, procedures, and protocol.

Respond as appropriate to personal calls from other health center units.

Perform first aid, call emergency personnel (dial 911), and/or call family members at request of residents.

Review resident service plans for assisted living and memory care.

Encourage residents to participate in scheduled facility activities, assisting in getting to activities as needed.

Assist in post‑mortem care as instructed.

Participate in oversight and management of medication delivery system, including timely order entry, receipt, tracking, and MAR documentation.

Notify pharmacy and supervisor of issues and seek to resolve them in a timely manner.

Promote the physical, personal, and emotional well‑being of each resident and strive to maintain their ultimate goal of returning to maximum level of self‑care and independence when possible.

Understand residents’ Advance Directives and follow their wishes.

Ensure effective communication is utilized by community staff with residents’ families concerning residents’ care.

Work well with others and take direction from management, while taking initiative for conflict intervention and resolution for residents, families, employees, and others as needed.

Other duties as assigned.

Qualifications

Graduation from an accredited nursing school and a current state license for LPN or RN is required.

1+ years of nursing experience preferred; new graduates are welcome.

CPR or BLS certification preferred.

Demonstrated proficiency in digital tools and applications, including Microsoft Office (Outlook, Excel, Word), web‑based platforms, electronic health record systems, and applicable HR systems.

Working Conditions

Position may require walking or standing for extended periods; may occasionally need to stoop, kneel, crouch, or crawl, as well as climb or balance.

Communication and written information interpretation skills required.

May need to lift up to fifty (50) pounds independently, and up to 200 pounds with assistance.

Must follow infection prevention and control protocols, including PPE when necessary.

Actively participate in staff activities fostering teamwork, unity, and morale.

Required to stay current on all training and ongoing education initiatives, pursuing self‑improvement opportunities.
