Outpatient Licensed Practical Nurse – LPN LVN Join to apply for the Outpatient Licensed Practical Nurse – LPN LVN role at Fresenius Medical Care.

Responsibilities

Provide direct patient care for assigned patient(s).

Perform all technical aspects of dialysis procedures as prescribed, administer and monitor treatments, and document parameters on dialysis flow sheets.

Participate in the implementation and evaluation of patient care, reporting new or unusual incidents and patient condition changes.

Assist in educating patients and families about end‑stage renal disease, dialysis therapy, diet, and medications.

Assist with emergency operational procedures, administer intravenous medication, and perform pre‑, interim, and post‑treatment reviews.

Initiate basic CPR measures in case of cardiac or pulmonary arrest.

Safely operate all dialysis‑related equipment, troubleshoot when necessary, and ensure a clean, safe, and sanitary environment.

Maintain effective relationships with external and internal stakeholders, respond to service issues, and assist other staff members.

Attend staff meetings, participate in care‑plan meetings, and in‑service classes.

Assist in coordinating the care plan with physician, dietitian, and social worker.

Qualifications and Experience

Graduate of an accredited School of Practical Nursing (LPN, LVN).

Current appropriate State licensure.

One year medical‑surgical nursing experience preferred.

Hemodialysis experience preferred but not required.

ICU experience preferred but not required.

Completion of a training course in the theory and practice of hemodialysis.

Completed CPR certification.

Must complete the Nurses Technical Training Program upon hire or soon thereafter.

Ability to provide coverage at area facilities during times of short‑staffing.

Meet the requirement of Ishihara's color blindness test.

Good verbal and written communication skills.

Physical Demands and Working Conditions The position includes desk and personal computer work and direct patient care with heavy lifting and moving of patients, including occasional movement of machines and equipment up to 200 lbs. The role requires frequent prolonged standing, frequent bending, and the ability to lift up to 30 lbs. Travel between facilities may be required. Exposure to infectious materials may occur. Reasonable accommodations may be made.
