Physical Therapist, 32 hours, Day shift, HomeCare, Thompson, CT Day Kimball Health is hiring for a part time Physical Therapist to join our HomeCare Department in Thompson, CT.

Location: Thompson, CT

Schedule: 8:00 a.m. - 4:30 p.m.

Hours: 32 hours per week

Weekends: every 3-4 as needed

Physical Therapist Job Summary Under the supervision of the HomeCare Rehabilitation Supervisor, organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential. You will also discover a culture of teamwork, professionalism, and mutual respect, and—most importantly—a fulfilling career.

Physical Therapist Job Responsibilities

Evaluates and treats patients as prescribed by referring physician. Reviews physician's referral and patient's condition and medical records to determine physical therapy treatment required.

Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device. Tests and measures patient’s strength, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and record findings to develop/revise treatment program.

Plans and prepares written treatment plan based on evaluation of patient data with realistic, objective, long and short-term goals and time frame.

Educates patient, family and significant others regarding condition, treatment and home program.

Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.

Orients, instructs, and directs assistants, aides and students.

Maintains professional growth by attendance of department meetings, inservice and educational conferences. Includes participation in hospital orientation and departmental and community inservice education and programs as directed.

Assists with the planning and implementation of new programs.

Physical Therapist Education and Experience Requirements

B.S. Degree and graduate from an accredited school of physical therapy required.

Connecticut license or Connecticut license eligible required.

CPR Certification required.

Minimum of one year experience preferred.

Valid Connecticut Driver’s License and proof of current insurance coverage required.

Physical Therapist Knowledge, Skills, and Abilities

Uses common sense and special medical skills to care for the sick or handicapped.

Understands technical information from supervisors, charts, reference books, manuals and labels.

Ability to use Portable Electronic Device to record/maintain patient information pertaining to Plan of Care.

Works fast and accurately in an emergency.

Identifies and acts on priorities for smooth departmental operations.

Maintains discretion in handling confidential nature of information available.

Keeps up to date on trends and developments in the field of physical medicine and health services.
