More About Our Team Dignity Health East Valley Rehabilitation Hospital is a joint venture between Dignity Health and Lifepoint Rehabilitation. How You’ll Contribute As a

Clinical Liaison , you will play a vital role in facilitating the patient intake and admission process by performing on‐site clinical evaluations to determine the appropriateness of patients for rehabilitation. You will serve as the primary point of contact for referring facilities, patients, and families, effectively communicating treatment options, admission requirements, and patient expectations. You will also gather key information related to patient benefits and resources while building and maintaining relationships with external case managers, discharge planners, and payors. This role requires clinical licensure, strong assessment skills, and the ability to navigate electronic medical records. A background in sales, marketing, or formal sales training is preferred for success in this role.

Builds and maintains referral relationships to drive appropriate admissions to the hospital. Coordinates the intake and pre‑admission screening process, conducts on‑site clinical assessments to determine suitability for acute rehabilitation, educates patients/families on services and participation requirements, and serves as the primary external contact for case managers, discharge planners, physicians, and payors.

What We’re Looking For

Clinical Licensure (PT/OT/PTA/COTA/SLP/Respiratory Therapist/RN) required

Previous clinical experience preferred with demonstrated skills in clinical assessment and EMR systems.

Bachelor’s degree preferred

Marketing and Sales/Formal Sales training experience preferred

Valid driver’s licence and clean driving record
