Patient Admissions Counselor, SNF – Perdue Medical Center

Summary The SNF Patient Admissions Counselor’s position involves independent action and judgment in assessing prospective admissions and their financial eligibility. Works with resident and/or family members to obtain appropriate documents to ensure all available benefits are applied for and attained. This position assists the Marketing Admissions Coordinator with on‑site admission preparation prior to arrival of resident as well as any other applicable operational duties. The incumbent may attend management meetings and may be involved directly or indirectly around confidential management decisions.
Responsibilities
Answers inquiries from prospective residents and/or family members, community hospitals, etc., providing a variety of information regarding medical and financial eligibility and facility services.
Gives facility tours upon request.
Receives referrals, obtains medical clearance from Nursing Department, checks financial eligibility, assigns bed.
Prepares temporary face sheet.
Conducts complete admission process.
Interviews all residents/family members, explains admission requirements and financial policies and procedures.
Obtains and verifies necessary information for the completion of admission and financial files, including signatures on all legal documents.
Places forms in designated portions of resident medical record and financial file.
Creates Medical Record number in the system if resident is not already in the Jackson Health System.
Creates and prints MSP form to determine if resident will be covered by Medicare.
Enters resident in the system once they arrive at the facility.
Gathers all required data and completes all applications for benefits including Medicaid ICP, Level of Care, Level II, Social Security Disability, and Representative Payee.
Works closely with ICP Medicaid Applications unit, Immigration and Naturalization Services, Social Security Administration, etc. to investigate available assets, financial resources and other issues as they relate to eligibility for financial assistance.
Attends appointments on resident’s behalf.
Responsible for creating and managing resident’s financial file to assure completeness of the documentation in relation to all financial resources, applications for benefits, etc.
Reports all changes in benefits or payment status to SNF Business Office Manager, Social Service department, and Health Information Management for updating of records as required (e.g., census, medical record face sheet, etc.).
Conducts annual review of each resident’s financial status.
Applies for residency status as needed.
Applies for Medicare benefits as needed.
Completes Medicaid ICP annual renewals.
Performs all other related job duties as assigned.
Experience
Generally requires 3 to 5 years of related experience.
Education
Bachelor’s degree in a related field is preferred.
Credentials
Valid license or certification is required as needed, based on the job or specialty.
