JOB DESCRIPTION Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work 8AM-4PM in the Somerset County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require care in their homes. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.70) included.

Internal referral bonus: $5,000

Reports to: Assistant Manager

Job Qualifications/Experience:

Required

Current NJ RN License

BSN

NJ driver's license in good standing.

Preferred

Minimum of one-year experience in acute, subacute or home/hospice care setting

Experience using an EMR, excellent critical thinking and communication skills

Duties & Responsibilities:

Performs Patient assessments.

Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals

Develops the plan of care for the patient and family

Ensures appropriate utilization of services

Maintains visit productivity standards

Demonstrates knowledge of insurance reimbursement

Completes discharge assessments

Performs laptop transfer of data twice daily

Completes admission assessment and transfers within 24 hours

Completes re-visit documentation and transfers

Uses age appropriate teaching methods for patients and families

Evaluates teaching plan and revises as necessary

Documents patient education tools used

Assumes responsibility for Case Management.

Develops/implements/evaluates plan of care based on evidence-based knowledge

Revises care plan as indicated

Prioritizes care needs

Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines

Shows evidence of interdisciplinary communication

Performs HHA supervisions per standards

Ensures appropriate utilization of services

Follows scheduling process

Communicates progress toward goals with IDT

Attends and participates in IDT meetings every 2 weeks

Evaluates the plan of care and revises as needed

Maintains safe environment for patients and co-workers.

Follows infection control measures and reports infections

Demonstrates knowledge of pharmacology and safe medication management

Routinely reconciles medications

Maintains current medication orders

Attends monthly staff meeting.

Participates in PI meetings and activities/committees as requested.

Accepts other assignments as requested

Assumes responsibility for case management.

Prioritizes schedule to accommodate patient's needs

Documents case management activities.

Demonstrates knowledge of pharmacology and safe medication management

Routinely reconciles medications. Maintain current medication orders.

Participates in Performance Improvement Program.

Reviews health records and gathers data to support PI activities

Attends UR meetings as requested

Provides orientation/ mentorship to new employees

Cross trains to learn new skills and demonstrates a willingness to teach other

Assists with growing agency business.

Provides orientation to new employees

Accepts other assignments as requested

QUALIFICATIONS Required:

Graduate of an accredited Nursing Program- Associates Degree

Current state licensure as a Registered Nurse.

BLS certification.

Preferred:

Bachelor's degree in nursing.

All ASN must complete their BSN studies within five years of their start date.

Master's degree in nursing or related field.

1+ year of experience in Nursing or relevant area.

ACLS certification.
