– a full‑time, hourly position reporting to the Lead Case Manager in the Case Management Department. The role provides service coordination to individuals with a range of disabilities and supports them through the social services system, promoting self‑determination and a person‑centered approach. The Case Manager must coordinate services for clients from CADI, BI, and DD programs and often work with individuals who have mental health needs, physical health conditions, brain injuries, developmental or neurodevelopmental disabilities.

Primary Duties And Responsibilities

Service Coordination

Take a person‑centered approach to identify the needs and preferences of individuals on your caseload.

Find resources to meet the unique needs of each individual.

Complete referrals for services.

Ensure services are not duplicated.

Maintain regular contact with providers to ensure quality service delivery.

Manage payment authorization for service providers.

Support Plan Development and Maintenance: identify goals, needs, and preferences; develop an annual support plan that integrates necessary policy components; update the plan as needed throughout the year.

Advocacy: promote health, safety, and well‑being; encourage independence and respect the right for individuals to take risks; assist individuals in expressing preferences and having their voice heard.

In‑person Home Visits: maintain a minimum of two in‑person visits annually with all individuals, either in their home or other community locations; must be willing to drive throughout the greater Twin Cities and occasionally travel throughout the state.

Documentation: document case notes in an electronic record system; maintain case notes for all work throughout the day; include a detailed, subjective narrative of all tasks completed; include precise time of each completed task to allow for necessary billing of work; ensure integrity in all documentation completed.

Requirements

Bachelor’s degree in a human services related field (required by DHS).

Experience in disability services preferred.

Experience working with mental health, preferred for CADI case management.

Ability to demonstrate patience with individuals experiencing difficult circumstances.

Must have a reliable vehicle and an adequate driving record for travel to in‑person visits at client homes.

Proficiency with Microsoft Office Products (Word, Excel, Outlook).

Ability to maintain tracking systems for multiple tasks to meet varying deadlines.

Excellent written and verbal communication with clients and other professionals.

Flexibility to adapt to frequent process changes.

Self‑motivation to complete tasks independently.

Willingness to ask questions.

Experience, ability, and commitment to working with individuals and teams that are diverse across line of difference such as race, gender‑identity, sexual orientation, religion, ability, age, class, and immigrant status.
