Your Impact in This Role The Clinical Services Coordinator is a cross-department clinical leadership role responsible for coordinating care, services, and communication across the Housing Department and Outpatient Therapy Department. This role is ideal for a clinician seeking expanded responsibility, systems-level experience, and a clear pathway into supervisory or program leadership roles.
Core Responsibilities Clinical & Case Management Leadership
Provide individual and group therapy services to assigned clients.
Provide and coordinate case management services across housing and outpatient therapy programs
Serve as a primary point of contact for residents, including after-hours support as needed.
Conduct intake assessments, eligibility screenings, and program enrollment.
Coordinate care and ensure follow-through with referrals to mental health, medical, housing, educational, and social services.
Lead problem-solving and conflict-resolution sessions to remove barriers to progress.
Coordination & Integration Responsibilities
Coordinate clinical and case management services across housing and outpatient therapy programs to ensure continuity of care.
Serve as a liaison between Housing, Outpatient Therapy, and external service providers.
Support alignment of treatment plans, housing goals, and recovery supports.
Identify service gaps, barriers to engagement, and workflow challenges; communicate recommendations to clinical leadership.
Support program implementation efforts, including intake flow, referral processes, and service coordination.
Assist the Clinical Director with cross-program communication, problem-solving, and coordination as assigned.
Community Engagement & Growth
Conduct outreach to individuals, families, and community partners to promote Center housing programs.
Represent the organization at community events, meetings, and collaborative initiatives.
Assist with volunteer coordination to support programming, meals, supplies, and outreach.
Collaborate closely with clinical, administrative, and leadership teams.
Professional Development & Advancement
Participate in clinical supervision, team meetings, trainings, and case consultation.
Take initiative in learning program operations, best practices, and leadership competencies.
Contribute ideas to improve services, workflows, and client outcomes.
Demonstrate readiness for increased responsibility and growth within the organization.
Qualifications
Master’s degree in Counseling, Social Work, Psychology, or a related field.
Licensed or license-eligible within one year (LPC or LCSW).
Strong organizational, communication, and time-management skills.
Ability to work independently, take initiative, and collaborate within a team.
Valid driver’s license and ability to transport clients as needed.
