Job Description: Answers and directs incoming inquiries about Home Care Services, prioritizing customer service. Communicates, coordinates, and disseminates information in a timely and pleasant manner. Ensures prescribing physicians meet all criteria and submit requests for new physicians to be entered into the system. Processes referrals for Start of Care (SOC) within twenty‑four (24) hours of the call. Facilitates referrals for infusion therapy, coordinating financial reimbursement, medication dosage, frequency, and delivery time to the patient’s home. Maintains current knowledge of reimbursement guidelines and confers with Performance Improvement staff/supervisors as needed. Completes insurance verification and relays information to appropriate staff, accepting referrals based on knowledge of accepted insurance/managed care plans. Documents all pertinent patient information in the electronic record, including code status and infection control precautions. Ensures physician coverage for patient care and proper completion of insurance intake forms for nurses to reference for subsequent visits. Submits monthly reports of referral sources data and admission statistics, including when hospital‑referred patients decline care. Functions as Patient Care Representative at hospital as requested. Assists with Performance Improvement functions as requested. Other duties as assigned.
The expertise and experiences you’ll need to succeed: QUALIFICATION REQUIREMENTS: Associate’s of Nursing (Required), Bachelor’s of Nursing, Basic Life Support - CPR Certificate (BLS) - RQI Resuscitation Quality Improvement, Registered Nurse (RN) - EV Accredited Issuing Body
Pay Range: $31.53 - $52.24
