Key Job Duties and Responsibilities The Direct Care Worker is required to provide direct care to community residents with a focus on activities of daily living. In addition, the employee will assist with housekeeping and laundry duties, mealtime service, and any other duties assigned by the Executive Director.
Assist residents in and out of bed and into wheelchair according to procedure; may require another aide or a gait belt.
Assist residents in bathing, dressing or undressing.
Assist resident to bathroom and provide peri‑care as needed.
Take and record vital signs of residents according to community schedule and policies.
Assist residents to dining area for meals; may be required to assist with feeding of residents.
Strip bedding and replace with clean linens per schedule or as needed.
Assist in personal grooming of residents.
Turn residents as needed when designated on care plan.
Clean bedpans and empty catheter bags.
Document and/or report information regarding residents as instructed; documentation must be completed prior to leaving the shift.
Maintain med room in a clean and orderly manner.
Give shift report on resident status as required.
Provide housekeeping/cleaning services as scheduled by the Executive Director for resident and common areas.
Provide laundry and linen service as required, delivering resident laundry as designated.
Perform minor maintenance and servicing on community equipment; inventory of supplies.
Monitor compliance with state and federal requirements for blood‑borne pathogens, hazardous materials, infection control, and fire safety.
Monitor compliance with residents’ rights.
Pass medications and give treatments per physician orders and community policies.
Perform simple dressing changes to wounds.
Notify RN of medications needing refill.
Maintain a clean and organized med cart.
Employee must be available to work rotating weekends and holidays.
Essential Job Duties / Physical Requirements
Regular worksite attendance.
Complete at least 12 hours of in‑service education approved by the community, Department of Human Services, and Department of Inspections and Appeals.
Complete Relias assignments on time.
Complete skills check‑off with the RN annually.
Develop and maintain good working relationships with co‑workers and other departments.
Basic computer skills.
Complete vehicle driver requirements if business use of motor vehicle.
Read, write, speak, and understand English.
Work beyond normal hours and on weekends/holidays when needed.
Assist in evacuation of residents during emergencies.
Perform essential functions with or without accommodation.
Use transfer/gait belt or another employee’s assistance for lifts over 25 lbs; aware of body movements that may occur naturally during routine duties.
Primary physical requirements:
Lift up to 10 lbs: constantly; 11‑25 lbs: frequently; 26‑50 lbs: frequently; >50 lbs: frequently.
Carry up to 10 lbs: constantly; 11‑25 lbs: occasionally; 26‑50 lbs: occasionally; >50 lbs: rarely.
Reach above shoulder height: frequently; at shoulder height: constantly; below shoulder height: constantly.
Push/Pull: constantly.
Hand manipulation: grasping: constantly; handling: constantly; torquing: occasionally; fingering: frequently.
Twisting, bending, crawling, squatting, kneeling, crouching, climbing, balancing required.
Relate to ill, disabled, elderly, emotionally upset, and occasionally hostile residents.
Use prosthetics as needed to ensure full performance of position.
