Social Worker – Bachelors

Position Summary The Licensed Social Worker promotes positive morale throughout the facility. The Social Worker works with residents, family/significant others and other team members to outline goals of stay at admission, the plan to meet those goals and discharge as appropriate.
Responsibilities
Ensure resident adjustment to rehabilitation or long‑term care by performing psychological screenings, initial plan of care, introduction to services and procedures, and completing all necessary paperwork including MDS.
Go over Resident Rights with all new admissions.
Ensure admission agreement completion.
Collaborate with families, Business Office and Medicare/Medicaid as a liaison to assist residents with applications and qualification for sub‑acute/long‑term financial assistance.
Schedule interdisciplinary team meetings with medical staff, residents and family members to plan care and resolve concerns. Write reports and ensure implementation.
Demonstrate understanding and provide guidance and counseling to residents and their families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
Coordinate discharge services—plan and acquisition of appropriate follow‑up care.
Qualifications
Long Term Experience: 1–3 years minimum.
Bachelor's Degree in Social Work, Psychology or Related Field.
Social Work Licensure (LSW, LCSW, or LICSW).
