Essential Job Duties / Responsibilities
Serve as primary point of contact for incoming and outgoing patient referrals. Triage referrals, gather necessary information, ensure timely processing and assignment to appropriate providers.
Facilitate communication, collaboration, and coordination of care. Coordinate appointments, referrals, transitions of care between primary care, specialists, hospitals, and other healthcare settings, ensuring seamless transitions and continuity of care.
Schedule patients utilizing coordinated provider list (CPL), make necessary arrangements related to the appointment, notify patients of appointment information: date, time, and location.
Use web-based insurance platforms to generate referral authorizations.
Effectively communicate the physicians/clinicians needs or outstanding items to patients.
Follow all referrals through to completed appointment and obtain all documentation related to appointment, uploading into the organization’s medical record system for physician review prior to PCP follow-up appointment.
Ensure any missed external appointments are rescheduled and communicated to the PCP.
Address referral-related phone calls from patients, providers, etc. Complete and address phone messages in a timely manner.
Provide extraordinary customer service to all internal and external customers.
Perform other related duties as assigned.
Pay Range $17.0 - $24.26 hourly. The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
