The
Referral Coordinator
plays a critical part in ensuring seamless coordination between patients, referring providers, and our internal clinical staff. The ideal candidate is detail-oriented, organized, and comfortable communicating with both patients and healthcare professionals.
Key Responsibilities: Serve as the primary point of contact for processing all incoming and outgoing referrals Verify insurance coverage and referral requirements to ensure appropriate scheduling Contact patients to provide appointment information and guide them through the intake process Coordinate with internal clinicians and external providers to support timely follow-up and continuity of care Maintain accurate and up-to-date documentation of referral activity within the EHR system Monitor referral trends and escalate delays or barriers to the appropriate manager Collaborate with the Patient Access and Clinical teams to improve efficiency and support overall access to care Track, assign, and report on referral and scheduling metrics to monitor trends, support workload distribution, and ensure team accountability Maintain consistent communication with key referral sources, providing scheduling updates and resolving issues as needed Partner with the Success and Operations teams to streamline workflows and strengthen relationships with referring providers Identify opportunities to enhance the patient and partner experience and contribute to ongoing process improvement Make periodic outreach calls to referral sources to share appointment availability and reinforce engagement Other tasks/duties as assigned
Requirements
Qualifications: High school diploma or equivalent preferred Associate's degree or higher in healthcare administration, health sciences, or a related field preferred Experience in healthcare settings or referral coordination can be considered in lieu of formal education Prior experience in a healthcare coordination, scheduling, or front desk role preferred Strong interpersonal and communication skills Proficiency with EHR systems; experience with PrognoCIS is a plus Exceptional organizational skills and attention to detail Ability to manage competing priorities in a fast-paced environment Professionalism, reliability, and a team-oriented mindset
