Shift: 08:30 AM – 05:00 PM 5 days in a week With Alternate Weekends
Pay Range: $45 - $50/hr. on W2
Job Description:
Duties: · Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients' nutritional needs and status and provides follow-up care. · Interviews patients and/or their families to obtain history of food habits and preferences. -Interprets pertinent laboratory, physical and social data in the medical record, as it applies to nutrition care. · Provides nutrition education and records patient's diet and diet instructions and the patient's understanding of the diet in the education section. -Modifies patient menus as needed.
