HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION Location:
Plymouth, NC – 100% Onsite
Counties Covered:
Martin, Washington, Tyrrell, and Hyde
Pay Rate:
$57.30/hr + mileage reimbursement ($0.43/mile)
Schedule:
Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
No weekends or on-call requirements
SUMMARY AND HIGHLIGHTS The Home Health Physical Therapist provides high-quality patient care through comprehensive evaluations, personalized treatment plans, and ongoing collaboration with interdisciplinary care teams. Contract assignment with potential for permanent hire based on attendance, performance, and business needs

401(k) with company match available to full-time employees after 1 year of service
RESPONSIBILITIES Perform physical therapy evaluations and treatments based on physician referrals
Develop, implement, and adjust individualized treatment plans
Establish measurable goals to maximize patient independence and functional outcomes
Reassess patient progress regularly and modify care plans as needed
Communicate patient needs and progress with providers, care teams, and families
Recommend and assist with adaptive equipment to enhance rehabilitation outcomes
Maintain timely and accurate clinical and billing documentation in compliance with regulations
Supervise Physical Therapist Assistants and Aides per state practice acts
Participate in staff development, meetings, and continuing education
REQUIREMENTS Education:
High School Diploma or GED required
License:
Active North Carolina Physical Therapist license (compact licenses accepted)
Attendance:
Mandatory attendance during the first 90 days
Skills:
Strong communication, documentation, and clinical assessment skills
Preferred:
Experience with Homecare Homebase (HCHB); comfort providing care across multiple counties
