Base pay range: $45.08/hr - $57.69/hr.

Department: MFS Patient Management. Location: Millard Fillmore Suburban, Williamsville, NY. Work type: Full-Time. Scheduled work hours: 8a-4p. Shift 1.

Job Description This position will coordinate continuity of care in collaboration with the interdisciplinary team. Ensure clinical standards of care such as clinical practice guidelines and patients’ care plans are met. Serve as liaison to ancillary and utilization review services and act as a communication link between health team members. Manage home referrals. Collaborate with Social Worker to address discharge barriers, coordinate patient/family education, and ensure documentation (advanced directives, agency choice) is completed according to standards. Attend unit shift reports, staff meetings, and interdisciplinary rounds as needed.

Education and Credentials Bachelor’s degree in Nursing. Current NYS registration as a Professional Nurse. Certified Case Manager (CCM) preferred.

Experience

At least 3 years acute care, medical/surgical nursing experience.

Minimum of 2 years in discharge planning, case management, and home care.

Knowledge of Utilization Management.

Proficiency with computer applications related to health care, MS Word, Excel, Outlook.

Working Conditions Essential: Weight requirement – Light (up to 20 lbs).
