Position Summary
Provide Case Management services and support to people who have a disability and/or medical needs that require assistance with Activities of Daily Living (including bathing, dressing, toileting, eating, and mobility). The primary objectives include establishing positive working relationships, exploring options and growth opportunities, creating and monitoring Person‑Centered Plans of Service (POS), meeting state‑mandated requirements, and ensuring that services meet standards. Responsibilities
Advocate on behalf of clients; provide resource information and education to increase self‑determination. Collect eligibility information and documentation; meet with clients or authorized representatives. Plan, facilitate, develop, and continually update POS and Person‑Centered POS tools. Research, identify, refer, and coordinate resources to achieve POS outcomes; assist with transitions. Monitor POS, conduct follow‑up activities, conduct on‑site visits, obtain progress information, complete Reportable Events, and re‑apply for programs as needed. Adhere to SCI policies, achieve performance benchmarks, maintain HIPAA records, and use tracking and time‑management tools. Competencies

Bachelor's or Master's degree in Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling, or a related field. Valid driver’s license and insured vehicle. Experience working with people with disabilities and person‑centered planning is preferred. Physical Demands
Standard office environment; mainly stationary work (>95% time). Ability to operate a computer, mouse, keyboard, copy machine, printer, and mobile device. Occasionally move office equipment (10‑25 lb). Must have adequate vision and be able to operate a motor vehicle. EEO Statement
