Position Full‑time Assistant – responsible for planning, implementing, and evaluating the Connections activity program for residents in Assisted Living, Memory Care, or Skilled Nursing settings.
Responsibilities
Enter resident information in multiple systems.
Attend care plan meetings with families and residents.
Collect resident profiles.
Assist in planning, developing, organizing, implementing, and evaluating the Connections activity program for the community.
Requirements
High School Diploma. Preferred Bachelor’s Degree in Social Work, Therapeutic Recreation, Psychology, Gerontology, or related field.
Minimum 21 years old with a valid state driver’s license. Preferred previous experience in healthcare or service related setting.
Knowledge of physiology and psychology of older adult.
Certification in CPR and First Aid.
Must have weekend availability.
