Overview HealthDrive is currently seeking a Licensed Independent Clinical Social Worker (LICSW) to work part‑time (2 days per week) in nursing facilities across the Cape Cod, MA area (work available in North Chatham and South Yarmouth). Opportunity for $40K+ per year (prorated based on schedule).

HealthDrive delivers on‑site behavioral health, primary care, dentistry, optometry, podiatry, and audiology services to residents in long‑term care, skilled nursing, and assisted living facilities.
Responsibilities The Therapist will provide behavioral health services to the residents of nursing homes, assisted living facilities, and long‑term care facilities within an assigned territory.
Essential Functions
Complete psychosocial (initial) assessments.
Provide individual and group therapy.
Develop behavior plans for residents.
Recommend behavioral/therapeutic interventions to nursing home staff.
Enter patient information and required clinical documentation in EMR system.
Perform other duties as assigned.
Mobile model treatment of patients requires daily travel to skilled nursing facilities.
Qualifications
Master's degree from an accredited college or university.
State professional LICSW license.
Valid driver’s license.
Ability to work independently on a daily basis.
Excellent written, verbal, interpersonal, and organizational skills.
Ability to use email, the internet, and to learn other healthcare related software.
